Called in KY at 1:15pm, Passport Health plan automated message asking me to fill out a health risk assessment form. Shows up as unknown in caller ID.
They called me from the number 8779030082 No message left. Not calling them back. Waste of my time. I do not know who this is nor do I answer calls from people I don't know. If you don't leave a message I won't call you, and more than likely if I don't know you I won't call you back.
